BWH Clinical Labs: Specimen Collection

Specimen Collection

The ability to obtain quality test results that accurately reflect a patient's condition depends on the proper collection, processing, and transport of specimens to the clinical laboratory. Entries in the Online Test Dictionary list collection requirements for individual specimens and tests. Please follow these requirements carefully when collecting a particular specimen. Deviation from the criteria may result in rejection of the specimen or may jeopardize the accuracy of any results. In particular, please adhere to the following criteria:
  • The patient must be correctly  identified and prepared  prior to collecting the sample.
  • Insure that specimens are appropriately transported to the laboratory.
  • Please be certain that you submit at least the minimum volume required for all tests ordered on a specimen. Such action averts having to recollect specimens, as well as the possibility of having inadequate specimen to perform ordered tests. Should we discover that we have not received sufficient sample, we will notify the outpatient practice. The comment "NSQ" will appear in place of the test results. Should the incorrect sample arrive for testing, we will hold the sample, notify the practice of the discrepancy, and request a recollection of the specimen.
In the Admitting Test Center and the outpatient phlebotomy areas:
  • Keep labels and requisitions with the patient so they will not be mixed with other patient labels.
  • Verbally identify the patient.
  • Check the requisition against the labels.
  • Label the tubes or containers.
  • Insert tubes in, or attach tubes to, the requisition before the patient leaves the blood drawing chair.
