Procedure for the release of blood & blood products

Blood or any of its products may be released by the blood bank to a physician, nurse, escort messenger, or any authorized carrier who presents the original copy of the blood product requisition form (#0515110) or its substitute to the blood bank. Blood products for one patient only will be released per carrier trip. Exceptions can be made for up to two patients per carrier trip.
  • Blood Bank Specimens: Specimens must be drawn in a 10mL lavender-top tube unless otherwise specified. Tubes with serum separators (Corvac/tiger-top tubes) should NOT be used.

  • Specimen Collection Requirements: All specimens must be properly labeled at the bedside with:
    • Patient's name
    • Medical record #
    • Location
    • Date and time
    • Initials of the blood drawer.
    The patient must be positively identified before the draw -- wristband check or identification by a physician or nurse.

  • Causes for specimen rejection: Test(s) will not be performed if the specimen is:
    • Improperly labeled.
    • Drawn in a Corvac tube.
    • Hemolyzed or grossly contaminated.

  • Turnaround Time: The computer will indicate when ordered blood products and tests are ready, and if a final processing step is required before issue of the product.

  • Request Forms for Products/Tests: Forms must be legible and must state the:
    • Patient's name
    • Medical record number
    • Location
    • Physician
    • Date
    • Product and/or test(s) desired

  • Neonatal Intensive Care Unit (NICU): Requirements may be different for ordering, specimens, and collection. Consult the NICU policy manual.
